Becoming a New Patient

If you are ready to start your journey with us, please follow these steps
1. You may schedule a new patient visit from our website or call 563 224-2212
2. Please have copies of your medical records at your appointment. Consider getting new blood work before our visit. With this information, we will spend your visit time more wisely. Our fax is (563) 202-2911
3. Fill out intake forms on the website or come 15 minutes early to fill out at the office on the day of our appointment
Initial visits
Your initial visit will likely be 45-60-minutes and includes:
-Retailed history and targeted medical examination
-Review of medical records
-Development of an integral treatment plan tailored to your particular endocrine condition. During this process, I work with you in evaluating causes of your symptoms and discuss possible treatment options
-You are a full partner in this process. No other patients are scheduled during your appointment time
Price of visits, payment at the time of visit
15-minute visit: $80
30-minute visit: $160
45-minute visit: $240
60-minute visit: $320
Cancellation of Appointments
– If an appointment is cancelled with less than 1 day’s notice, you will be charged $25
– We understand that life happens so if you need to reschedule or cancel an appointment, please call our office within the cancellation policy guidelines.
Payment Options
– cash, checks, Paypal, or credit cards (Mastercard, Visa, American Express or Discover) at the time services are rendered.
